1. Membership Tiers & Monthly Credits

• Each membership tier is billed monthly at its designated rate.
• The full monthly payment is converted into account credit that can be used toward services and treatments.
• Credits may be applied to any eligible service unless otherwise specified.

2. Rollover Policy (12-Month Cap)

• Unused credits roll over month-to-month.
• Credits may accumulate for up to 12 months (1 year) from the date they are issued.
• Any unused credits older than 12 months will automatically expire.
• Credits will continue to accumulate as long as the membership remains active and in good standing.

3. Membership Perks
   All membership tiers include:

• Complimentary B12 injection
  • Must be used within the active billing cycle
  • Must be used during a scheduled appointment (no walk-ins solely for B12)
  • Non-transferable and cannot be banked or exchanged for cash or other services

Additional perks vary by tier:

• Tier 1: Discount on Botox treatments
• Tier 2: Increased discount on Botox + discount on dermal fillers
• Tier 3: Highest discount on Botox + highest discount on dermal fillers + thread lift

(Exact discount percentages are determined by membership level.)

4. Usage of Credits

• Credits may be used toward services only unless otherwise specified.
• Credits cannot be used on promotional pricing or special event pricing unless stated.
• Credits cannot be redeemed for cash or transferred to another individual.
• Promotions, credits and membership discounts may not be combined unless explicitly stated.

5. Billing & Payments

• Memberships are billed automatically each month on the original signup date.
• A valid payment method must be kept on file.
• Failed payments may result in suspension of membership benefits until resolved.

6. Minimum Commitment

• All memberships require a 3-month minimum commitment.
• After the initial commitment period, memberships will continue on a month-to-month basis unless canceled per policy.

7. Cancellation Policy

• Cancellation requests must be submitted in writing prior to the next billing cycle.
• Upon cancellation:
  • Any unused credits will remain available until their 12-month expiration date
  • No new credits will be added after cancellation
  • Membership discounts and perks will be immediately discontinued

8. Membership Changes (Upgrades & Downgrades)

• Members may upgrade at any time, effective immediately.
• Downgrades require a 30-day notice and will take effect the following billing cycle.

9. Freezing Memberships

• Memberships may be frozen for a limited time under special circumstances.
• During a freeze period:
  • Billing is paused
  • No new credits are added
  • Existing credits retain their original expiration timeline (12-month cap still applies)

10. Appointment & No-Show Policy

• Standard cancellation policies apply to all members.
• Late cancellations or no-shows may result in a fee or deduction of credits.

11. Policy Changes

• We reserve the right to modify membership pricing, benefits, and policies at any time.
• Members will be notified in advance of any significant changes.

12. Agreement
    By enrolling in a membership, clients agree to all terms and policies outlined above.
